When students sign and submit this form, it authorizes The Graduate School to collect fees associated with cataloging and copyrighting of the dissertation. Cataloging the dissertation is underwritten through a mandatory $72.00 fee. If a student chooses to copyright their work, there is an additional $70.00 charge for filling the copyright. Students submit this form with a personal check directly to The Graduate School for processing. The check should be made out to SUNY@ Buffalo. Alternatively, students may opt to fill out and submit the form and attach no payment, which will result in the Student Response Center posting these fee charges to the student’s account.
